Privacy Policy

ACM Dental Care, P.C., doing business as Dentist on Madison ("Dentist on Madison" or "the practice"), operates the website dentistonmadison.com for its dental office at 274 Madison Ave, Rm 202, New York, NY 10016. This Privacy Policy explains what information the website collects, how the practice uses it and who processes it on the practice's behalf.

The website's interactive features run on Logika, the platform the practice uses for its contact form, appointment requests, text chat, Second Opinion inquiries, insurance check, financing inquiries and voice assistant. This policy covers the website and those online features. It does not describe how the practice keeps the records of your dental treatment; ask the office if you have a question about those records.

Forms and requests

The contact form asks for your first name, last name, email address and phone number, with space for an optional message. It also asks for your permission for the practice to respond by phone call or email, and it cannot be sent without that permission. When you press Send, the details go to the practice through Logika. The permission covers a reply to your inquiry. It is not consent to marketing or to text messages, and sending the form does not book an appointment.

The appointment request flow asks which service you are interested in, your name and a phone number or email address. You can also add a preferred day and part of the day, how you would like to be contacted, your insurance plan and a note for the team. The front desk uses these details to contact you. A request is not a confirmed appointment: the visit is arranged only after the office contacts you and agrees the details with you.

The Second Opinion flow asks you to choose a treatment topic and give a phone number or email address. Your name and a written question are optional. You can also choose to upload supporting documents or images for the practice to consider, and these files may contain sensitive health information. Uploaded files are stored privately in Logika application storage, where authorized practice personnel can access them. They are not published on the website. Uploading is optional; please send only material that relates to your question and that you have the right to share.

The Check Your Insurance form asks for your name, phone number, email address and insurance carrier, and lets you add your member ID if you wish. The practice uses these details to compare your carrier with the carriers it lists as accepted and to follow up with you about your plan. A match does not verify your eligibility or benefits and does not confirm coverage. Your benefits depend on your plan.

The appointment request, the Second Opinion request and the Check Your Insurance form also offer two optional checkboxes for text messages, described under "Text messages" below.

The practice offers payment plans through Cherry. The financing inquiry form asks for your contact details, an optional question and your permission for the practice to respond. The inquiry goes through Logika to the practice, not to Cherry. It is not a credit application, and sending it does not mean you are approved for financing.

Text chat

The text chat asks for your name and phone number before a conversation starts. Replies come from an AI assistant: your messages are processed by Logika and the AI-model provider it uses, and the replies are generated from the practice's public information. Practice staff can read the conversation and may follow up with you. The assistant is not a dentist. It cannot examine you, make a diagnosis or give individual treatment advice. The text chat is separate from the voice assistant and does not use your microphone. Closing the chat does not delete messages the practice has already received.

Voice assistant

The voice assistant starts only when you press its button in the website header. Your browser then asks for permission to use your microphone, unless you have already allowed it. During a session, your speech is processed by Logika's voice service and the voice-model provider it uses so that the assistant can respond. The assistant also receives information about the page you are on, such as the current page and the sections and actions available on it. Its answers are automated. It is not a dentist and does not diagnose or give clinical advice.

You can end a session at any time and withdraw microphone permission in your browser settings. Ending a session stops further transmission from your microphone; it does not undo processing that has already taken place. You can always use the contact form, phone or email instead.

Sensitive information and emergencies

Outside the Second Opinion flow and the optional member ID field in Check Your Insurance, please share only what the office needs to reach you or answer your question. Do not enter medical records, detailed health histories, Social Security numbers, insurance member ID numbers or payment card details in the website's other features. If you are unsure how to share something sensitive, call the office first. None of the website's features is an emergency service. For a medical emergency, call 911.

Technical information, cookies and browser storage

Cloudflare hosts the website and serves its pages, images and fonts from the website's own address. The fonts are hosted with the site rather than loaded from an outside font service. Like any web host, Cloudflare receives technical information needed to deliver and protect the site, such as your IP address, the time of each request, browser details and the page requested. When you use a Logika feature, Logika and the providers behind that feature receive similar technical information and may keep technical logs to run and secure their services.

The website does not currently use analytics. Its pages do not load Google Analytics, Google Tag Manager or any other analytics tool, and the site does not use advertising or remarketing cookies. The text chat may use your browser's session storage to keep the current conversation available in that tab. The Cookie Policy explains this in more detail. If the practice decides to add analytics, it will update this policy and the Cookie Policy before any analytics is enabled.

Links to social media and other outside services open those services, which have their own privacy notices.

How the practice uses and shares information

The practice uses the information you provide to reply to you, return calls, handle appointment requests, review Second Opinion questions and documents, answer insurance and financing questions, and keep a record of its communications with you. Logika and the providers that support hosting, application storage, AI chat replies and voice processing handle the information needed to provide those services. Authorized practice personnel can see the requests, messages and documents relevant to their work. Information may also be disclosed where the law requires it.

Information you enter in the website's features is not sent to analytics or advertising services. A request for a call or an email reply is not permission to send you marketing.

Text messages

The appointment request, the Second Opinion request and the Check Your Insurance form each include two optional checkboxes, both unchecked when the form opens: one for texts about your request and appointments, and one for marketing texts such as offers and promotions. You can send any of these forms without selecting them. If you select a box, the practice may text you only about the kind of messages you chose. Giving your phone number in a form, chat or voice session without selecting a box is not consent to receive texts.

The practice's text messaging is still being set up; the sending number will be listed in the SMS Terms and Conditions before texts begin. You can stop texts at any time by replying STOP, as the Opt-out page explains. Your mobile number and your consent to receive texts are not shared with other companies for their own marketing. Service providers that deliver the practice's messages process your number and the messages only to provide that service. The Opt-in page shows the exact wording of both checkboxes.

Keeping, correcting and deleting information

How long information is kept depends on the type of record, why it was collected and the recordkeeping rules that apply. Closing a page does not erase a request you have already sent. You can contact the practice to ask what information is connected to your website inquiry, to correct it, to ask for it to be deleted or to change how the office contacts you. The office may need to confirm your identity first, and legal or patient-record obligations may limit what can be deleted. This policy does not limit any rights you have under New York or federal law.

Parents or guardians should contact the office on a child's behalf rather than having the child use the website's forms, chat or voice assistant. No online service can guarantee complete security, so please do not send sensitive health information by ordinary email.

Changes to this policy

The practice will update this policy when the website's information practices change. The date at the top shows the current version. The Terms and Conditions cover general use of the website, and the Accessibility Statement explains how to report a barrier.

For privacy questions or requests, contact the practice. Include a way to reach you, and leave health details out of your message.
