These terms apply to your use of dentistonmadison.com. The website is operated by ACM Dental Care, P.C., doing business as Dentist on Madison ("the practice"), a dental office at 274 Madison Ave, Rm 202, New York, NY 10016. It lets you learn about the practice and its services and get in touch with the office. It does not replace a dental examination, an individual treatment plan or instructions from your dentist.

General information, not dental advice

Service descriptions on the website are general information. A treatment listed on the site may not be suitable for you or available at a particular visit, and a dentist needs to examine you before recommending care. Photographs and descriptions do not promise a particular result. Patient reviews, wherever they appear, describe the experiences of the people who wrote them.

The text chat and the voice assistant are automated AI assistants. Their answers are not a diagnosis, a treatment recommendation or the opinion of a dentist. If an answer seems unclear or does not match what the office has told you, ask the office before relying on it for a health decision.

Appointment requests and other inquiries

An appointment request tells the office which service you are interested in and when you would like to come in. It does not reserve or confirm an appointment. The front desk calls you back, and the appointment is set only when you and the office agree on the details. Sending a contact form, a chat message or a Second Opinion inquiry does not confirm a visit either, and it does not mean a dentist has reviewed your information.

Check Your Insurance compares your insurance company with the carriers the practice lists. A listed carrier is not a guarantee of coverage. Your benefits depend on your plan, so confirm them with your insurer and the office. The practice offers payment plans through Cherry; the financing inquiry form sends your question to the practice. It is not a credit application and does not mean financing is approved. Fees and payment arrangements for your care are discussed with the practice directly.

Using the website responsibly

Provide accurate information that you have the right to share. Do not impersonate anyone, send abusive or unlawful material, interfere with the website, or try to access it or the systems connected to it without authorization. The contact form, chat and voice assistant are not the place for detailed health records, insurance member ID numbers or payment card details. If you want the practice to look at documents, use the Second Opinion flow.

Privacy and connected services

The contact form, appointment requests, text chat, Second Opinion flow, Check Your Insurance tool, financing inquiry form and voice assistant run on Logika, the platform the practice uses for these features. The Privacy Policy explains how information sent through them is handled, and the Cookie Policy covers browser storage. Asking for a call or an email reply is not consent to marketing or to text messages. Texts are sent only to people who select an optional text-message checkbox in the appointment request, the Second Opinion request or the Check Your Insurance form; the Opt-in, Opt-out and SMS Terms and Conditions pages explain how that works. Emergencies

Website features are not emergency services. For a medical emergency, call 911. For a dental emergency, call the office during office hours.

Governing law and changes

These terms are governed by the laws of the State of New York. Nothing in them waives rights or protections you have under applicable law. Website content and features may change, and the date at the top identifies the current version of these terms. Any agreement about your treatment or its cost is made separately with the practice.
